Please ensure that you have the following documents before you initiate to fill this form.
- Scanned images of the biographic data page and relevant pages confirming your departure dates of your passport.
- Those who leave for overseas training should attach the documents given below to the relevant cage of the departure register.
- A letter from the primary employer approving overseas study leave. (Ministry of Health, University, Armed forces etc.)
- Please download, complete, and upload the attached document below according to the nature of the overseas training appointment.
