Examination Application Form

The steps to be followed when applying for examinations at the PGIM are as follows

  1. PG trainees shall refer the examination calendar for the current year published by the PGIM
  2. Download relevant examination circulars 
  1. If an applicant meets all the prerequisites outlined in the issued circular by the deadline, they are eligible to download and complete the examination application form. 
  1. Details on how to fill in the application form are as follows:
    • Part A – Should be filled by the candidate( Fill it as a word doc and print)
    • Part B – Should be recommended by the Consultant/Head  of the Institute
    • Part C – Should be recommended by the DGHS. Instead of the Director General of Health Services (DGHS), authority is delegated to the head of the line ministry institutions or the Regional Director of Health Services (RDHS) to recommend and forwarded the application to the Director/PGIM.
  1. Candidates shall pay the relevant examination fee online.
    • Payment instructions are given in
    • Once the payment is made, the candidate shall receive a payment receipt to his/her email. (please note that Application fee and Registration fee are included in the Online payment)
  1. From the circular letter no 048/2021 all examination applications and other documents are collected through a Google Form. The link has been posted in the relevant examination circular. All applicants should have a Gmail Account to log into the google form. PGIM trainees have to use the email account created by the PGIM. You should attach your exam application with all the relevant documents as a single PDF document since this portal does not allow you to attach multiple documents.
  2. Late applications, incomplete, hand written and applications sent via emails will not be accepted.
